The practice principal, Doctor Johanna Kovatz, will not renew her contract as visiting medical officer at the Crookwell District Hospital.
She will continue to provide services alongside Dr Heidi Ehmann and Dr Rebecca Scott and the network of professionals at the centre.
Dr Dalia Henss and Dr Amy Chahal have also left the practice, making way for Dr Ivan Liu. He will be taking on patients next week.
Dr Liu is completing his placement to finish his specialisation in general practice.
To alleviate the widespread doctor shortage in rural areas the centre has become a practice to facilitate practitioners completing their training.
Both Dr Henss and Dr Chahal finished their term.
